Wood's lamp or clinical assessment of depth, plus a history of triggers — pregnancy, contraception, heat exposure and any previous laser that made it worse.
- 02Topical phase
Prescribed topical therapy started and titrated. This does most of the work; the laser is an adjunct, not the main event.
- 03Low-fluence toning
Added where the physician judges it useful, at deliberately low energy. Melasma can rebound darker under high-energy settings.
- 04Maintenance
Daily broad-spectrum sun protection plus a review interval. Stopping maintenance is the usual reason melasma returns.
