01The core difference (in 1 line)
Sculptra is a COLLAGEN BIOSTIMULATOR — its job is to make your fibroblasts build new collagen in deficit zones (temples, mid-face, jawline). Profhilo is a BIO-REMODELLING agent — its job is to flood the dermis with hybrid HA so the skin holds water and behaves like younger tissue.
Sculptra answers the question 'I've lost volume.' Profhilo answers the question 'my skin doesn't behave like it used to.' These are different complaints and call for different molecules.
02When Sculptra is the answer
Signal: you look at photos from 5-10 years ago and the FACE SHAPE has changed — temples have sunk, upper cheeks are flatter, jawline less defined. That's volume loss from age-related fat-pad atrophy + bone resorption.
Filler can mask it short-term; Sculptra rebuilds the underlying scaffold so the face shape returns naturally.
- Sunken temples — visible at rest, not just when smiling
- Flat or hollow mid-face — light no longer catches the cheekbone
- Marionette lines from volume loss, not skin laxity
- Age 35+ with diffuse volume loss
03When Profhilo is the answer
Signal: face SHAPE looks fine but the SKIN looks dull, crepey, or doesn't bounce back. The 'glass skin' you used to have is gone.
Photos under harsh light show fine etched lines and dullness even though there's no volume deficit.
- Crepey skin on the cheeks or under-eye area
- Dull, sluggish-looking skin even with good skincare
- Loss of natural bounce — pinch test takes longer to recover
- Skin laxity on neck or hands (Profhilo treats these too)
04Can you do both?
Yes — and for patients in their 40s and 50s this is often the smarter plan. The standard combination protocol at Waleerat:
Phase 1 (months 0-2): Sculptra — 2 vials spaced 4-6 weeks. Rebuilds the structural deficit.
Phase 2 (months 3-4): Profhilo — 2 sessions spaced 4 weeks. With volume already returning, Profhilo polishes the skin quality on top.
Result at month 6: face shape restored AND skin behaves better. We do NOT do them in the same session — they target different layers and want different aftercare.
05Comparing cost per plan, not per unit
Waleerat lists Sculptra at ฿25,000 / 1 catalogue bottle and Profhilo at ฿22,999 / 1 catalogue syringe (2 ml); a 3-syringe Profhilo package is also listed. These are catalogue unit prices, not treatment totals.
How many bottles or syringes a plan uses, and how often it is repeated, depends on the assessment, so a yearly cost cannot be worked out from unit prices alone. Ask for a written total that names the product, quantity and number of visits.

06Common questions
I have a ฿25K budget — which one should I do?+
Start from the concern, then the written plan total. For volume loss, the listed unit is one Sculptra bottle (฿25,000 / 1 catalogue bottle), which targets ONE specific zone (temples OR mid-face — not both).
For skin quality, it is one Profhilo syringe (฿22,999 / 1 catalogue syringe (2 ml)). A unit price is not a treatment total, so ask for the total for your plan before booking.
If unclear which concern is primary, the free physician consult before booking is exactly for this conversation.
I'm in my 30s — is Sculptra appropriate yet?+
Usually no. Patients in their 30s rarely have meaningful volume loss; what they often have is early skin-quality change (less bounce, occasional dullness).
Profhilo fits that signal better. Sculptra typically becomes appropriate from late 30s onwards, when fat-pad atrophy starts showing in photos.
Does Profhilo replace Sculptra in some way?+
No — they treat different problems. Profhilo cannot rebuild structural volume; Sculptra cannot improve surface hydration.
The marketing of 'biorevitalisation' sometimes blurs this, but mechanically they're different. If you have volume loss AND skin quality loss, the answer is both, not one substituting for the other.
If I do Sculptra first, when do I add Profhilo?+
We typically add Profhilo at month 3 after the second Sculptra vial — that's when collagen rebuild has begun and the surface skin is ready to be polished. Doing Profhilo TOO early (week 1-4 after Sculptra) is fine clinically but you waste the visible-result window because you can't tell which product is doing what.
