01Why anatomy decides dose
Sculptra's PLLA microparticles need to land in the deficit AND reach a critical mass to trigger meaningful collagen rebuild. Different facial zones have different volumes of deficit and different surrounding tissue depth — so 'the right dose' is geography, not arithmetic.
A 1-vial course mapped to one specific temple is sub-therapeutic if spread across both cheeks. The same vial deployed correctly at one temple can be transformative.

02Case 1 — Temples (early 40s)
Presenting: a patient in her early 40s, mostly bothered by sunken temples visible in side-angle photos. Mid-face and cheeks were age-appropriate but the temple hollows photographed as 'tired' under harsh light.
She had previously tried 2 cc of HA filler at another clinic — improvement lasted ~10 months and she didn't want to keep refilling.
Plan: 1.5 vials of Sculptra mapped specifically to the bilateral temple zone — 0.75 vial per side, deep periosteal placement. Single session, ฿37,500 total.
The temple is one of the highest-visible-return zones for Sculptra because the deficit is well-defined, the surrounding tissue is thin, and the patient sees the change in their own mirror within 12 weeks.
- Week 1: diluent swelling settled in 72 hours

03Case 2 — Sunken mid-face (late 30s)
Presenting: a patient in her late 30s, lost 12 kg over the past year and reported 'looking gaunt.' Mid-face fat-pad atrophy was the main complaint — upper cheeks flattened, light no longer caught the cheekbone, and friends had started commenting that she looked tired. She wanted natural recovery, not 'filled' look.
Plan: 2 vials of Sculptra across the bilateral upper cheek + mid-face zone — 1 vial per side, supraperiosteal placement with cannula. 2 sessions spaced 6 weeks apart. ฿50,000 total.
Mid-face is the most common reason patients book Sculptra at Waleerat (we estimate ~60% of cases) because the volume loss is what photographs most obviously aging in this demographic.

04Case 3 — Sagging cheeks (early 50s)
Presenting: a patient in her early 50s with lower-face sag — cheeks drooping toward the jowls, jawline definition lost. She'd considered thread lift but wanted to start with a less aggressive option.
Important: lower-face sag is partly volume loss AND partly skin laxity. Sculptra addresses the first, not the second.
Plan: 2.5 vials over 3 sessions, mapped across mid-face + jawline + lower-cheek transition zone. ฿62,500 total.
We told the patient upfront that the result would be 'restored structure + better light-catch' but NOT a thread-lift level of repositioning — for that we'd need to add threads at month 6 if she wanted more. This honest framing is critical: Sculptra under-promised is better than Sculptra over-promised.
She also committed to the 5-5-5 massage protocol after each session.

05Pattern across the three
Three patterns we see repeatedly across hundreds of Sculptra cases:
First: visible-return-per-vial is highest in the TEMPLES, mid-tier in the MID-FACE, and lowest in the LOWER-FACE. This is because deficit volume vs Sculptra dose ratio is most favourable for temples.
Second: patient satisfaction at month 6 correlates more with EXPECTATIONS SET AT CONSULT than with vial count. Patients told 'this will rebuild structure' are happier than patients told 'this will lift your face.' Third: the 5-5-5 massage protocol matters more than patients expect — cases that skipped it had visibly worse results.
