01The core difference (in 1 line)
Sculptra is a COLLAGEN BIOSTIMULATOR — its job is to make your fibroblasts build new collagen in deficit zones (temples, mid-face, jawline). Profhilo is a BIO-REMODELLING agent — its job is to flood the dermis with hybrid HA so the skin holds water and behaves like younger tissue.
Sculptra answers the question 'I've lost volume.' Profhilo answers the question 'my skin doesn't behave like it used to.' These are different complaints and call for different molecules.
02When Sculptra is the answer
Signal: you look at photos from 5-10 years ago and the FACE SHAPE has changed — temples have sunk, upper cheeks are flatter, jawline less defined. That's volume loss from age-related fat-pad atrophy + bone resorption.
Filler can mask it short-term; Sculptra rebuilds the underlying scaffold so the face shape returns naturally.
- Sunken temples — visible at rest, not just when smiling
- Flat or hollow mid-face — light no longer catches the cheekbone
- Marionette lines from volume loss, not skin laxity
- Age 35+ with diffuse volume loss
03When Profhilo is the answer
Signal: face SHAPE looks fine but the SKIN looks dull, crepey, or doesn't bounce back. The 'glass skin' you used to have is gone.
Photos under harsh light show fine etched lines and dullness even though there's no volume deficit.
- Crepey skin on the cheeks or under-eye area
- Dull, sluggish-looking skin even with good skincare
- Loss of natural bounce — pinch test takes longer to recover
- Skin laxity on neck or hands (Profhilo treats these too)
04Can you do both?
Yes — and for patients in their 40s and 50s this is often the smarter plan. The standard combination protocol at Waleerat:
Phase 1 (months 0-2): Sculptra — 2 vials spaced 4-6 weeks. Rebuilds the structural deficit.
Phase 2 (months 3-4): Profhilo — 2 sessions spaced 4 weeks. With volume already returning, Profhilo polishes the skin quality on top.
Result at month 6: face shape restored AND skin behaves better. We do NOT do them in the same session — they target different layers and want different aftercare.
05Cost-per-year math
Per-year cost is the right comparison because the duration is different:
- Sculptra 2-vial course (฿50,000) lasts ~2.5 yr → ฿20K/yr
- Sculptra 3-vial course (฿75,000) lasts ~2.5 yr → ฿30K/yr
- Profhilo 2-session course (฿28,000) lasts ~6 mo → ฿56K/yr
- Combined (Sculptra 2 vials + Profhilo 1×/yr) ≈ ฿34K/yr
