01Why 'skin quality' is the new aesthetic conversation for patients 35-50
Five years ago, the standard Bangkok aesthetic protocol for 35-50 was Botox plus HA filler. Forehead lines softened, cheek projection restored, nasolabial folds erased.
The category called this the 'liquid facelift' and patients booked it on a 6-month cadence.
That protocol works — but it's solving an old problem. The new problem for patients in their late 30s and 40s is not loss of volume or wrinkle depth.
It's loss of skin quality: the change in elasticity, hydration retention, and surface texture that makeup and skincare can't compensate for. Patients describe it as 'my skin looks tired' or 'foundation doesn't sit right anymore'.
Bio-remodelling treatments are the category response. Profhilo (IBSA, Italy) is the most-studied and most-shipped — it's been in market since 2015 and has a substantial European clinical evidence base.
Korean and domestic competitors exist (Lenisna, Restylane Skinboosters) but Profhilo remains the reference standard for the category.
The patient profile that benefits most: 35-50, has tried HA filler or considered it, doesn't actually want more volume, struggles with hydration and makeup-sit-on-skin issues, and has 4-8 weeks of patience to wait for the full result. 02The science: how bio-remodelling actually works
Profhilo is 32mg/ml of hyaluronic acid — extraordinarily high concentration — delivered as a non-cross-linked hybrid cooperative complex. The 'non-cross-linked' part is the critical engineering choice.
Cross-linked HA (every traditional filler) creates a gel that resists tissue spreading and stays where it's injected. Non-cross-linked HA flows freely under the skin and distributes across a wider area.
When Profhilo is deposited at the standard injection depth (sub-dermal), it spreads across roughly 3-4cm² per injection point. Over 24-48 hours it slowly hydrolyzes back into low and high molecular weight HA fragments that signal to fibroblasts to upregulate collagen and elastin production.
The clinical result: more elastic, more hydrated, denser skin in the treated zone.
The 5 BAP (Bio-Aesthetic Points) injection technique is patented to IBSA. Five precise sub-dermal injection points per side of the face — chosen to avoid major nerves and arteries and to maximise the area covered by the freely-spreading gel.
Total injection time: 10-15 minutes. No deep injection, no extensive massage, no facial mapping like with filler.
What Profhilo does not do: it does not add volume to a hollow cheek, it does not lift a sagging jawline, it does not erase a deep nasolabial fold. Those are different problems requiring different tools (filler, biostimulator, RF respectively).
Profhilo treats skin quality only — the substrate the other treatments sit in.
03Profhilo vs traditional HA filler — different tools, different jobs
Cross-link is the engineering choice that determines everything else. Cross-linked HA filler stays in place because the HA chains are chemically bonded into a stable gel — you can inject 1ml into a cheek and have 90% of it still in that exact location three months later.
Non-cross-linked Profhilo deliberately doesn't stay in place — it spreads and signals to fibroblasts.
Treatment-zone difference: HA filler is focal (1-2cm³ deposit creates a specific contour effect at a specific location). Profhilo is distributed (one vial covers the full face across 10 injection points).
You can't substitute one for the other any more than you can substitute foundation for moisturiser.
Volume change: HA filler is a volume-restoration tool. The Allergan Juvederm Voluma Z trial measured ~0.8ml retained at the treated site at 24 months.
Profhilo's mechanism deliberately routes around volume — the freely-spreading gel hydrolyzes within 4 weeks and is fully gone by week 8. What persists is the collagen and elastin the fibroblasts produced in response.
When you actually need both: a 45-year-old patient with mid-face volume loss AND poor skin quality (the more common case than either alone) gets HA filler in the cheek deep plane plus Profhilo across the lower face for skin density. The two treatments complement, don't compete.
The order matters: filler first to establish the substrate, Profhilo 2-4 weeks later to improve the skin layer.

04The 2-session starter protocol and what to expect at each visit
Session 1 (60 minutes total clinic time, 15 minutes actual injection): photo documentation, 30 minutes of topical anaesthetic, 5 BAP injection points per side at the standard sub-dermal depth. You'll feel mild pressure rather than pain at each injection point.
Small papules (raised bumps) appear immediately at each point — these are the deposited gel and resolve in 4-6 hours.
Days 2-7 after session 1: papules fully resolved. Most patients see no visible change yet.
The fibroblast response is starting but isn't surfaced yet. This is the 'did I just spend money on nothing' phase — entirely normal.
Weeks 2-4: first hydration glow becomes visible. Skin reflects light differently.
Makeup sits better. Photographs (especially close-up phone selfies) look cleaner without filter or retouching.
This is the photo most patients send to friends.
Session 2 (4 weeks after session 1): repeat protocol. The same 5 BAP per side at the same depth. The combined effect of 2 sessions delivers the full collagen-stimulation response.
Weeks 4-8 after session 2: peak result. Elasticity improvement noticeable to the patient (skin springs back faster when pressed).
Hydration retention measurable in the morning (less compensating with moisturizer at the bathroom mirror). The 6-month maintenance window starts counting from this point.
05What ฿2,500 first-vial means and what's NOT included
The intro price covers one 2ml vial of Profhilo — the full Italy-imported pack from IBSA — delivered as Session 1 in the starter protocol. It applies to new Profhilo patients only (patients who've had Profhilo at another clinic don't qualify).
The discount is structured to remove the price-friction of trying a new treatment category, not as a recurring promotion.
Included: physician consultation, 30 minutes of topical anaesthetic, the 5 BAP injection technique, post-session photography, and a follow-up review at week 4 (the natural time to assess and schedule session 2).
NOT included: Session 2 is at the standard rate of ฿14,000 — your physician will quote this transparently at the consult. We do not bundle the 2-session pricing into the intro because some patients prefer to evaluate the result after session 1 before committing to session 2.
Both pathways are fine; the price is the same either way (2 vials at the maintenance rate).
