Thermage, Ultherapy, Oligio, Ultraformer — what actually separates the four lifting devices

In one paragraph

Patients almost never ask this as an open question. They ask "Ultherapy or Thermage?" — as though the four devices were four brands of the same thing, and one of them had to win. They are not the same thing. Two of them use focused ultrasound and two use radiofrequency, and that difference decides which layer of your face receives the energy. Once you know which layer your own concern lives in, the shortlist usually narrows itself, and the honest answer for a fair number of people is that no device is the right first step at all. This guide sets out the mechanism, the depth, the session time and the starting price for each, then covers what none of them can do. Specifics always depend on your anatomy and your skin — results vary from person to person, and a consultation is what turns this into a plan.

01The real split is ultrasound versus radiofrequency

Four names, two technologies. Ultherapy and Ultraformer are focused ultrasound platforms.

Thermage and Oligio are monopolar radiofrequency platforms. Everything else about how they feel, how long they take and what they are aimed at follows from that one fact.

Focused ultrasound behaves like a lens. Energy passes through the surface without depositing there and converges at a set depth, creating small, discrete coagulation points.

The depth is chosen by the transducer — commonly 1.5 mm, 3.0 mm and 4.5 mm. The deepest of those reaches the SMAS, the fibromuscular layer a surgeon repositions in a facelift.

That is why ultrasound devices are described in terms of lift.

Radiofrequency behaves like a bulk heater. Current meets resistance in the tissue and warms a volume of dermis fairly evenly across the area under the tip, rather than at one chosen plane.

Neither mechanism is a better one. They answer different questions, and the useful question is not which device wins but which layer your own concern lives in.

02The ultrasound pair — Ultherapy and Ultraformer

Both deliver micro-focused ultrasound to set depths including the SMAS. They differ in platform, coverage and price rather than in principle.

03The radiofrequency pair — Thermage and Oligio

Both are monopolar radiofrequency, heating a volume of dermis rather than a chosen plane. The practical difference is track record versus newer engineering.

04Matching the device to the concern

Read this as a starting point for the consultation, not as a prescription. Two people with the same photograph can need different plans, because skin thickness, fat distribution and bone support differ.

  • Jawline and jowl definition, a lower face that has begun to descend — the SMAS is involved, which points to the ultrasound family first.
  • Crepey texture, fine lines, skin that reads slack rather than fallen — a dermal question, which points to radiofrequency.
  • Both at once, which is common after the mid-forties — a combined plan usually makes more sense than choosing one.
  • Body areas as well as face — Ultraformer is the platform among these four that covers both.
  • Comfort or a tight schedule as the deciding factor — the MPT platform runs shorter sessions than the others here.

05Why they are often combined rather than compared

Because ultrasound and radiofrequency deposit energy at different depths, they are not mutually exclusive, and treating them as rivals is what makes the choice feel harder than it is. A common approach is to address the deeper structural layer with focused ultrasound and the dermal layer with radiofrequency, spaced rather than stacked on the same day.

Sequencing is a clinical decision, not a package. It depends on which layer is doing more of the work in your face, what you have had before, and how much recovery time you actually have.

06What none of these devices will do

This is the part most comparison articles leave out, and it is the part worth reading twice.

None of these devices removes skin. Where laxity is significant enough that there is true excess, an energy device tightens what is there without addressing the surplus, and the result disappoints in a way no number of sessions fixes.

That is a surgical question, and if that is what we see at consultation we will tell you rather than sell you a course of treatment.

Ultherapy or Thermage — which one should I pick?+
They are aimed at different layers, so the honest answer depends on which layer your concern lives in rather than on which device is stronger. Ultherapy is focused ultrasound reaching the SMAS, so it is aimed at lift; Thermage is monopolar radiofrequency heating the dermis, so it is aimed at tightening and texture. If a jawline has begun to descend, the ultrasound family is the usual starting point. If the complaint is crepey or slack-looking skin, radiofrequency addresses it more directly. Many patients ultimately do both.
What is the difference between Ultherapy and Ultraformer if both are HIFU?+
Both are micro-focused ultrasound working at set depths including the SMAS, so the principle is shared. They differ in platform and coverage: Ultraformer is the CLASSYS system and is the one of the four that also treats body areas, while Ulthera Prime offers broader coverage at SMAS depth for a more pronounced lifting effect. Session length and starting price differ too — Ultraformer MPT runs 30–50 minutes from ฿14,999, Ulthera SPT 45–60 minutes from ฿18,000.
Is Oligio just a cheaper Thermage?+
No — Oligio X starts higher, from ฿40,000 against ฿28,000 for Thermage FLX. Both are monopolar radiofrequency. The differences are in engineering and track record: Oligio X is a newer Korean platform from Daeyang Medical with dual G-mode and X-mode operation and skin-contact cooling 2.25× cooler than the previous generation, while Thermage FLX has the longer clinical history. Which of those matters more is a reasonable thing to discuss at consultation.
How soon would I see a change?+
Collagen remodelling is gradual rather than immediate. The response builds over roughly two to three months after treatment, and how much builds varies between individuals — age, skin thickness and degree of laxity all affect it. Anyone quoting you a guaranteed result or a fixed timeline is overstating what these devices do.
Can I have ultrasound and radiofrequency treatments together?+
They are frequently combined, because they act on different depths. They are normally spaced rather than performed on the same day, and the sequence depends on which layer is contributing more to your concern, what you have had previously, and your recovery time. That sequencing is a clinical decision made at consultation.
Will a device work instead of a facelift?+
Not where there is significant excess skin. These devices tighten and stimulate the tissue that is present; they do not remove skin. If the degree of laxity means a surgical approach is what would actually address it, we will say so at consultation rather than recommend a course of device treatment that will not meet your expectation.
