Melasma sits deeper and behaves differently from freckles or sun spots, and it punishes aggressive treatment: high-energy laser can rebound it darker. The program is deliberately gradual — prescribed topical therapy does most of the work, low-fluence laser toning is added where the physician judges it useful, and sun protection is treated as part of the treatment rather than advice given at the door.

Wood's lamp or clinical assessment of depth, plus a history of triggers — pregnancy, contraception, heat exposure and any previous laser that made it worse.

Prescribed topical therapy started and titrated. This does most of the work; the laser is an adjunct, not the main event.
