01What is a biostimulator — and how it's different from a filler
A biostimulator is an injectable that triggers the body's own collagen production rather than adding instant volume. The result builds over 2–6 months as new collagen forms, lasts 12–24 months depending on the ingredient, and produces a more 'natural' look because the volume IS your own collagen — not gel.
Hyaluronic acid filler (Juvéderm, Restylane, Belotero) is the opposite: instant volume on injection, gradual resorption over 6–18 months. Different problem, different tool.
Patients who want immediate cheek volume for a wedding next month need filler. Patients who want their face to look better in six months without an obvious change need a biostimulator.
The ingredient inside the biostimulator determines what tissue layer it acts on and what indication it fits. Five ingredients are clinically relevant in Bangkok 2026: PLLA (Sculptra), stabilised HA (Profhilo, Skinvive), PDLLA (Juvelook, Belluxi has a PCL+PDLLA hybrid), CaHA (Radiesse), and PDRN (Rejuran's salmon-DNA fragments).
PCL (used in Ellansé and Belluxi) is the sixth and overlaps mechanically with PLLA but lasts longer.
02The 5 ingredients — what each actually does at the cellular level
PLLA (Poly-L-Lactic Acid). The Sculptra molecule.
Slow-degrading polymer microsphere — when injected at deep dermis / subcutaneous level, it triggers a controlled foreign-body response that produces type I collagen around each microsphere. Effect builds over 3–6 months, lasts 24+ months.
Best for structural volume loss in the temples, cheeks, mid-face — where the issue is actual deficit of tissue, not skin quality.
Stabilised HA (Profhilo, Skinvive). Hyaluronic acid that's been thermally cross-linked but at a lower density than fillers — produces a 'spreading' gel that distributes through the dermis rather than sitting at the injection point.
Triggers fibroblast activation that synthesises new collagen + elastin + endogenous HA. Effect builds over 1–2 months, lasts 6–9 months.
Best for skin quality, hydration, fine-line texture — where the issue is skin envelope, not volume.
PDLLA (Poly-DL-Lactic Acid). Microsphere form of PLLA that's been refined to a smaller, more uniform particle.
Juvelook is the most-prescribed in Asia. Smaller microspheres mean a gentler collagen stimulation pattern — less risk of nodules, smoother integration, suitable for shallower injection.
Effect builds over 2–4 months, lasts 12–18 months. Best for fine texture, early laxity, neck and decollete where PLLA's larger microspheres would be too aggressive.
CaHA (Calcium Hydroxylapatite). The Radiesse / HarmonyCA molecule.
Mineral particles suspended in a CMC gel — the gel gives immediate volume on injection, then resorbs over 3 months while the calcium particles stimulate collagen formation. Hybrid behaviour: immediate AND long-term result.
Lasts 12–18 months. Best for jawline definition, dorsal nose bridge, mid-face structural support.
PDRN (Polydeoxyribonucleotide). The Rejuran molecule.
DNA fragments derived from salmon sperm that bind adenosine receptors on skin cells and stimulate fibroblast activity, wound healing, and cellular repair. Distinct mechanism from collagen-stimulating microsphere injectables.
Effect builds over 4–8 weeks of weekly sessions, lasts 3–6 months between cycles. Best for acne-scar texture, sensitive skin, post-laser repair, rosacea-prone skin.
03Decision tree — match the biostimulator to the indication, not the brand to the budget
Patient profile A — late 40s+, structural volume loss in temples, mid-face hollows, deflated cheeks. Recommendation: PLLA (Sculptra).
The deep tissue restoration needs PLLA's larger microsphere and 24+ month duration. Skin-quality biostimulators won't address structural volume loss.
Patient profile B — 35–50, skin texture, dullness, fine lines on cheeks + neck + back of hands. Recommendation: stabilised HA (Profhilo) or PDLLA (Juvelook).
Both improve skin quality through different mechanisms — Profhilo via hydration + fibroblast activation, Juvelook via microsphere-stimulated collagen rebuild. Profhilo is faster (~6 weeks), Juvelook lasts longer.
Patient profile C — 30s, fine-texture early laxity, glowy 'glass skin' aesthetic goal. Recommendation: PDLLA (Juvelook) or PDRN (Rejuran) depending on whether the goal is collagen rebuild (Juvelook) or cellular repair (Rejuran).
For Asian patients who want the Korean 'glass skin' look specifically, Rejuran has stronger evidence on skin-quality markers.
Patient profile D — acne-scar texture, post-laser tissue repair, sensitive/rosacea-prone skin. Recommendation: PDRN (Rejuran).
The DNA-fragment mechanism specifically supports cellular repair and inflammation reduction. Other biostimulators may be too aggressive for inflammation-prone skin.
Patient profile E — jawline definition, mid-face structural support, projecting nose bridge. Recommendation: CaHA (Radiesse or HarmonyCA).
The mineral particle + immediate volume hybrid is the right tool for structural projection. We sometimes layer this with PLLA for compound long-term volume.
04The 5-row comparison table — what we show at every consult
Mechanism: Sculptra = PLLA microsphere → collagen. Profhilo = stabilised HA → fibroblast activation.
Juvelook = PDLLA microsphere → fine collagen. Belluxi = PCL+PDLLA hybrid → multi-effect (clear pores + tone + lines).
Rejuran = salmon-DNA fragments → cellular repair.
Effect onset: Sculptra months 3–6 (slowest). Profhilo weeks 4–8 (fastest). Juvelook months 2–4. Belluxi weeks 6–12. Rejuran weeks 2–4 between sessions, cumulative across cycle.

05How biostimulators stack — with HIFU, filler, and each other
Most-common stack: biostimulator + HIFU / MFU-V. Sequenced 4–6 weeks apart, not same session.
The HIFU stimulates SMAS-level collagen while the biostimulator stimulates dermal-level collagen — different depths, complementary mechanisms. This is the strongest non-injectable+injectable combination for 45+ patients with both laxity and skin-quality concerns.
Second stack: biostimulator + HA filler. Different problems addressed simultaneously — biostimulator for the long-term collagen rebuild, HA filler for the immediate volume that the patient sees in the mirror tomorrow.
The patient leaves the clinic looking better immediately (filler) AND will look better in 6 months (biostimulator). Sequence: filler first (instant gratification), biostimulator at week 4.
Biostimulator + biostimulator combinations: less common, but used for layered indications. PLLA (Sculptra) + Profhilo addresses both structural volume AND skin quality — most patients in their 50s with budget benefit from this combination.
PLLA + Rejuran for patients with structural volume loss AND acne-scar texture. We spacing sessions 6–8 weeks apart to avoid concentrated inflammation.
What NOT to combine: PLLA + PDLLA in the same session (mechanism overlap, increased risk of nodule formation). Profhilo + non-cross-linked HA skin booster (mechanism overlap — redundant).
Rejuran + active steroid use (immune-modulation conflict).
06The realistic timeline — and why patients who judge results too early ALWAYS regret it
Week 1: nothing visible. The injection itself causes minor swelling that resolves in 24–48 hours, but no collagen has formed yet.
Patients who photograph at week 1 often think the biostimulator 'didn't work' — they're judging the wrong timeline.
Month 1: stabilised HA biostimulators (Profhilo, Skinvive) start showing visible texture improvement. PLLA, PDLLA, CaHA, PDRN are still building — nothing visible.
This is the most discouraging-looking phase for the longer-acting ingredients.
Month 3: PDLLA (Juvelook) and CaHA (Radiesse) start producing visible volume and texture improvement. PLLA (Sculptra) is still building. PDRN (Rejuran) cycle completes, skin-quality improvement is visible.
Month 6: PLLA (Sculptra) at peak effect — visible volume restoration in temples and mid-face. Profhilo treatment may need second cycle if the patient maintains the result.
Juvelook + Belluxi at peak — skin texture and collagen rebuild are at their strongest visible point.
Month 12–18: PLLA results still strong (24+ month duration). Other biostimulators may need maintenance.
We schedule maintenance at the natural decay curve of each ingredient — not before, not after. Premature 'maintenance' wastes the patient's budget; delayed maintenance lets the result fall further than necessary.
Why the 6-month after-photo discipline matters: patients who judge results at month 2 invariably book unnecessary 'topup' sessions that wouldn't have been needed if they'd waited for the natural collagen-rebuild curve to complete. The biology dictates the timeline.
Booking earlier doesn't accelerate the result; it just costs the patient more without changing the outcome.
